Nito was probably "alive" at some point.

It's possible that claiming the Lord Soul of Death made him into that skeletal being.

Stella, you get that sword he is holding by joining his Gravelord covenant. :)Though its not as big as the one he actually has in the game, if you run down to the catacombs at the start of a new character and get it, it's a really good starting weapon. :)
